The forms were stamped with official codes from the Utah Department of Health, and in no uncertain terms stated the forms would need to be complete in order to receive a birth certificate.

A new state law will allow Utah-born adoptees to get a non-certified copy of their original birth certificate without having to go to court, the Utah Department of Health announced Tuesday.
